Transcranial direct current stimulation (tDCS)

TDCS is a form of neurostimulation that uses a constant, low current delivered directly to the area of the brain of interest via small electrodes. When these electrodes are placed in the area of concern, the current induces current flow in the cerebral cortex, which can increase or decrease neuronal excitability in specific areas to alter brain functioning.

  • As early as 1794, Giovanni Aldini had already evaluated the effects of head currents in his own body, and by 1804 he had already reported the successful treatment of patients suffering from depression
  • 1930s: TDCS was abandoned as other forms of neurostimulation were considered to have more potential (Woods et al., 2010.)
  • 1960: TDCS was found to affect sebum excitability (Woods et al., 2010)
  • Studies after 2012: TDCS is now better understood and applied under conditions with good imaging and stimulation techniques, (Nitsche et al., 2012 onwards)

Bipolar stimulation

  • When a stimulus has two electrodes, an anode and a cathode, it is referred to as bipolar electrical stimulation. This, in turn, defines the +/- polarity near each stimulating electrode. tDCS is the most common application when using bipolar montages. This has two potential drawbacks.
  • Anodal stimulation of an electrode is inevitable with a cathodal stimulation electrode (and vice versa), without a reference electrode (safety issue)
  • Requires a very large electrode covering a large cortical area to produce the electric field Disadvantage is that it is not focused (low precision)
